What is Sciatica?

The Sciatic nerve is the longest single nerve in the body largely responsible for connecting the spinal cord to the hips, legs, and feet. The Sciatic nerve runs from the lower spine through the buttock, hips, legs and feet. Sciatica pain can present in a variety of ways and severities depending on the injury. Pain can range from a dull ache but is more commonly a sharp or searing pain that may also present as a pins and needles sensation, numbness, or weakness in the legs.

What Causes Sciatica?

There are a number of conditions that cause pressure on the Sciatic nerve and the pain associated with Sciatica. These include:
  • Herniated disks of the lumbar spine (lower back) are the most common cause of Sciatica pain. These discs are pads of cartilage that sit between the vertebrae of the spine and act as shock absorbers that cushion the vertebrae and provide flexibility. When these discs wear down they can't provide the cushioning needed. Sometimes, the discs will press on a nerve root, causing pain in the legs and/or back, as well as weakness, numbness or tingling of the foot, leg and buttocks.
  • Spondylolisthesis is often the result of a degenerative disc disease and occurs when one vertebrae slides forward over another vertebrae causing the displaced vertebrae to pinch the sciatic nerve where it exits the spine which causes the pain associated with this form of Sciatica.
  • Lumbar spinal stenosis is caused when areas of the spinal channel (the protective sheath of the spinal cord within vertebrae) narrow within the vertebrae and this puts pressure on the spinal cord or nerve roots causing pain.
  • Piriformis syndrome occurs when the piriformis muscle (which connects the thighbone to the lower spine) becomes tight or spasms, putting increased pressure on the Sciatic nerve. The pain from this may be felt from the lower back down to the knees.
  • Trauma and injuries caused by car accidents, falls or other events can injure the lumbar and sacral nerves in the lower back and cause Sciatic pain.

Treatment

Home treatment.
  • Cold packs : Using cold packs for about 20 minutes several times a day can ease the inflammation placed on the painful area. 
  • Hot packs : Using a hot pack or a heating pad may help to reduce pain. If not try alternating hot and cold.
  • Stretching : Stretches that don't twist or jerk can provide necessary relief and can help to relieve some nerve root compression.
  • Over the counter medication - i.e. ibuprofen
